Mechanism of action
Evidence map: Alpha-GPC
After oral intake, alpha-GPC is cleaved in the intestinal mucosa to glycerophosphate and free choline and is thereby supposed to enlarge the supply of choline for acetylcholine synthesis [1]. In rats, it abolished the memory impairment induced by scopolamine and attenuated the drop in acetylcholine in the hippocampus and cerebral cortex, but not in the striatum [1]. Microdialysis measurements showed a dose-dependent increase in acetylcholine release in the hippocampus; radiolabeled alpha-GPC was converted to acetylcholine [2].
In humans, in a small randomized crossover study in six healthy men, choline and betaine in the blood rose after alpha-GPC to a similar extent as after other choline supplements; all water-soluble supplements at the same time rapidly raised trimethylamine N-oxide (TMAO), which gut bacteria form from choline [3]. In young and older volunteers, alpha-GPC enhanced the release of growth hormone after stimulation with GHRH, which fits an increased cholinergic tone [4]. It did not change inflammatory markers in the rat brain [5].
Clinically, alpha-GPC has been studied mainly in dementia. In a multicenter RCT of 261 people with mild to moderate Alzheimer's dementia, it improved ADAS-Cog, MMSE, and further scales over 180 days compared with placebo [6]. A meta-analysis found advantages mainly in combination with donepezil [7]; its last author has himself published studies on this combination (ASCOMALVA) [8]. Older reviews rely in part on uncontrolled studies [9].

Human studies
Alzheimer's dementia, 180 days
Measured: ADAS-Cog, MMSE, GDS, CGI
Result: ADAS-Cog −3.20 points with alpha-GPC vs. +2.90 with placebo after 180 days; significant differences on several scales.[6]
Human, randomised
Meta-analysis of cognitive disorders
Measured: Cognition, function, behavior
Result: With donepezil, cognition better (MD 1.72; 95% CI 0.20–3.25); alpha-GPC versus placebo or other agents MD 3.50 (0.36–6.63).[7]
Human, randomised
Cognition after a single dose in healthy men
Measured: Stroop, N-back, flanker
Result: Stroop total score better (d = 0.48–0.61); N-back and flanker without a difference. The author advises the sponsor.[11]
Human, randomised
Strength and vigilance after seven days
Measured: Jump power, isometric strength, psychomotor vigilance (PVT)
Result: No difference in vigilance and isometric strength; jump values better only at the lower strength.[12]
Human, randomised
Motivation in everyday life
Measured: self-reported emotional state (KOKORO scale)
Result: Trend toward more motivation, significant in the evening; no effect on anxiety.[13]
Human, randomised
Stroke risk in users
Measured: Stroke overall, ischemic, hemorrhagic
Result: After matching, higher stroke risk in users (aHR 1.43; 95% CI 1.41–1.46), dose-dependent; observational data, no causality.[10]
Human, non-randomised
Risks and warnings
Higher stroke risk (observational data)
In a cohort of 12 million people aged 50 and older, users had a stroke risk about 40% higher after adjustment for many risk factors, with a dose-response relationship. As an observational study, it does not prove a cause; reasons for prescription could play a role [10].
Human, non-randomised
Rise in TMAO
Alpha-GPC rapidly raised TMAO in the blood, as did other water-soluble choline supplements [3]. Whether this is clinically harmful is unresolved; the cohort study names TMAO as a possible mechanism [10].
Human, randomised
Lowering of TSH
At the higher of two strengths, thyroid-stimulating hormone (TSH) was significantly lower after seven days [12]. Significance unclear.
Human, randomised
Interactions
- Donepezil: Given as a combination in studies; the meta-analysis found better values with the combination than with donepezil alone [7].
Fact check: common claims
- partly supported
“Alpha-GPC increases acetylcholine in the brain”
In rats, more acetylcholine release in the hippocampus [1][2]; not measured directly in humans, only indirectly via choline in the blood [3] and growth hormone [4].
- partly supported
“Alpha-GPC improves focus and memory in healthy people”
A small, sponsor-related RCT found a better Stroop test but no effect in two further tests [11]; another RCT found no effect on vigilance [12].
- not supported
“Alpha-GPC increases strength and growth hormone”
More growth hormone only after additional GHRH stimulation [4]; in RCTs, no difference in growth hormone and strength after training [11] and in isometric strength [12].
- partly supported
“Alpha-GPC helps in Alzheimer's dementia”
One positive RCT over 180 days [6] and a meta-analysis with advantages mainly together with donepezil [7]; independent confirmations in large studies are lacking.
- not supported
“Alpha-GPC is harmless as a food ingredient”
A large observational study shows a higher stroke risk in users [10]; a cause has not been proven, but has not been ruled out either.

Open questions
- Is the higher stroke risk in the Korean cohort causal or due to the reasons for prescription?
- Does alpha-GPC achieve a measurably higher acetylcholine release in the brain in humans?
- Can the positive Alzheimer's findings be replicated in independent, large RCTs?
